Mrs. Harold is either Jess's paternal grandmother or a foster parent. We now know Jess was taken away from Sydney, right after birth, because Sydney didn't have the means to raise her. And until she does have the means and the right living conditions, she won't get custody of her daughter back. So, it's only natural Mrs. Harold suspects there's a material reason why Sydney is dating a doctor.
