I got news for you Aussies and non North Americans. It's a spoiler, but you guys are here knowing that.

The files that Syd saw at the end of Season 3 that upset her so much which had information about Jack seeming to have planned on Syd being an agent since she was born also had information that Jack asked for permission to execute Irina. Syd read that he was granted permission and the deed was done.

The good news is Sloane asked Jack if he told her the truth about. Jack said that he didn't tell Syd because she wouldn't be able to handle it. So Irina probably isn't really dead.
